Here are a few suggestions and observations during my playthough that might be worth taking a look at:
1. I built my settlement / gates and 2 days later a gang with 10 people and knives showed up. The result was predictable and I got wrecked. Only in the current lategame where you rund around with 6 well equiped and trained companions do you have a chance. Maybe built in a blocker that makes the event available much later in the game. Also: the rest of the settlement could help you out here as this is a survival event for them all. Maybe guards can be added to the equation? Doesn't have to be something fancy but a single calcualtion that says 1 of the attackers was taken care by one of your guards and 3 settlers hindered a second one forcing you into fighting a second wave might be worth it.
2. Some general info on how skills are acquired would be nice for beginners. You'll find out later when you deliberately check the numbers but a one liner in the help section would be helpful to know that you need to perform a job 101 times.
3. Training scavangers is annoying with them being able to die. Maybe remove the die options when you have to go look for them as soon as they acquire the scavenger trait?
4. I like maxing stats. I was pretty dissapointed to max out submission and then seeing the girls loosing the stat over time. Makes it also annoying when they deny you sexual stuff after you trained them up earlier. Maybe you could build in a system that lets you keep the stat maxed out? Maybe equipping a girl with a butt plug and a chastity belt could freeze the current submission value? Or just a similar job for the guests as the mistress that keeps the value up? Or a collar as a new item could do the trick.
5. Getting metal early in the game is very difficult as you only have scavengers that can do so. It took me almost 300 days to make the sanctuary available and metal was a very limiting factor until then. Maybe provide an other option to get some much earlier (iron ore + cole from the quarry + a smithy plan to build stuff at home?) or just make it available at another shop.
6. You can normaly get around having to sell girls to other people. The 4 blonde girls for Negan really annoy me. Wish there was another option. I also picked up the three girls Dom wanted me to kill/drag to him.
7. I assume the questline isn't finished but what's the exact advantage of sending girls to the glory hole? They make much less money than working on the streets.
8. It seems that offsprings come without any skills. Maybe Laura can do something there as training them up from 0 is a lot of work. It also makes them less valuable as some skills aren't learnable as of yet. Also wish there was a system for girls to acquire all traits. In my opinion anal slut trait should reduce horny gain from vaginal sex.
9. Jobs for slaves are very limited - either growing food or working on the streets. Maybe under the right conditions(high relationship, high submission) you can put them up in other jobs as well? Currently it just seems that all the good and useful jobs are reserved for guests and you can't freely decide what girls do as soon as your guest rooms are all occupied. Or you just put in a certain amount of rooms and let the player decide if they are guests or slaves.
10. Heat wave pretty much blasts your food prodution to oblivion. Maybe there could be a settlement upgrade that just halves production rates?
11. When you choose a custom NPC package it would be nice to see the criteria of the pack directly after the girl name e. g. Riley Reid: white, young, brown hair and if the package is already in use for another girl or not.
12. Maybe put the "Net change, produced(+) and consumed(-) excluding shop (1) " and other important info at the top (skills learned, births) so that you don't always have to scoll down if you're not interested in the less important information.
13. As soon as you get the shop materials are put in the storage. Which makes them unavailable when you're moving around and want to sell some stuff. 10 wolf pelts in the storage doesn't let you give them to Octavia when you're visiting her. You first need to get them manually from the storage. Logically it makes sense, gameplay wise it's annoying. Maybe it's worth thinking about fusing them together except there are different advantages I'm not aware of?
14. It would be nice to see your Progress to the Fighter Skill as well as initially check the corresponding number of girls when you sent them to fights instead of always having to manually select 1-3 NPCs (maybe with an option to pick girls with/without the fighter skill first - if the event even increases this specific stat).
15. Eating in the kitchen doesn't seem to have any effect as of now.
16. Blairs friend doesn't seem to visit anymore. Dunno if it's because I didn't participate once, it's because I have a maid assigned or a bug.
17. Being unforgiving/evil/without concience in this game pays caps by just selling NPCs as slaves. It would be nice to get some other benefits when you're good. E. g. not killing/capturing your attackers. Maybe some accumulative bonuses for your settlement?
18. Not sure about this one, but I brought some slaves to Negan and one of them had a gas mask equiped. Didn't look like I got the item back when she changed ownership. Don't know how items are generally handled when the NPC is killed/sold/removed by an event.
19. Maybe you could provide a few option to change Avatars for the female story NPCs (Blair, Eve) and make their Avatars available as Portraits for them and not just the random ones.
20. You should specify that the slaves Negan wants have to be female, questlog just says " Bring Negan twice 2 slaves and then visit his office. (80+ beauty and blonds)". I did find 2 guys with the specific values and it didn't work - what a shame....
21. Do chastity belts currently have any use? It seems they just prevent youself from fucking the girls vaginally - when you're the one having the corresponding key...
I hope this gives you a few new ideas or let you review some existing systems.

difficulty is from qty of humans at your base, at the end of the game when you wil lbe at 50 or more there will be huge qty of attackers. train, recruit companions faster and train them at evening 2 times each day near fire, train at underground arena, buy or find weapons and armor. craft.

But normally yyou just sell useless slaves without skills and use only useful.
Try bounty hunting, there sometimes slaves with good skills. Beautiful girls always at waterfall event.
10. Seriously i don't mind food production at all, it is useless, as soon as you get too much there will be event when you need to throw out food or your settlers will die from food poisoning.
I do this as soon as food hits 50-100 i buy up to 300
15. Eat food at kitchen only need if you was hungry(do not have enough food at the base), sleep after this and hunger rose and you need to replenish energy or health this way. It is really rare occurence.
16. It is rare events as well as other events, for wxample Eve visits for shower.
17. It is not in mechanics yet i think. IDK what Developer planning for this, or maybe ge done something after 0.54 version
20. I think Negan mentioned that he need girls, blonde, beauty...
