I have always been a fan of creampie and pregnancy tags, so it's a guarantee of having them in the game. But I'm still considering that the game should have a sex scene while the LIs are in pregnancy or not. And yes, there is a major route where you need to impregnate all the main girls to get it.

To your question. Does this game have a choice between cum inside or outside? probably yes, but it's just for the climax in sex scene; the LIs can still get pregnant if they don't use a pill or birth control after having vaginal sex with MC. (In real life girl still get a chance to be come pregnant even if you come outside.)

TLDR: Coming inside or outside does not matter if you choose to have protection, but if not, prepare for child support and the consequences.

I don't know what you mean by "planned" pregnancy but rather by "uncertain" pregnancy. Does it mean that there will be no sexual relations with a pregnant woman or that she will not look pregnant? That's a great idea, but I wonder if you pull out all time and then the girls still have pregnant. Does it make sense. Because no semen approaches egg, how does she get pregnant.

According to this site tags rules, it can mean 2 things.

The one that will be certain in the game is the impregnating one, but I'm still not sure that I should include the pregnant sex too or not because it's not one of my kind. If people support that idea, I may put it in the game(it's not much of a deal).

Pulling out does not make you safe from getting pregnant. When having sex, male and female will release pre-cum aka body fluid to make on going sex go smoother. Inside male pre-cum, there is a small amount of sperm, which, if they reach the egg by traveling through female fluid, can fertilize it. Also sperm can live up to five days inside vagina, so there a small chance to get pregnant even you pull out. Also if the sperm is too weak even 10 creampies can't impregnate single girl

Ahegao? Yes, likely. Mind break? not quite, but there is a LI's corrupted version of the sex scene (eh... you can imagine having sex with Yandere girl STL; well, it's still crazy anyway). Lactation? maybe, if many people support pregnant sex. Urination? hard to tell; it's not impossible but likely not going to happen unless a major group of followers agrees about it, which includes some conditions like not all LI have this scene and must be an acceptable scenario for me (not too gross, no drink, no humiliation, etc.).
