You gotta get the cell upgrades from socket, you can find her in the mines but I think you gotta go through a couple of her scenes first.

i wish the comfortable insertion would stay permanently on the npc we slept with

If you walk around the prison some npcs will interact with, some wanting to offer something to you or taking something by force.
The NPCs also walk around the prison and if you are the same room as them you click look around and mess with the NPCS in the room. It's random because you depend of the NPC pathing. Beside a small icon on the mini map, like a pickaxe that means the NPC is going to mine, you can't predict their behavior.
Also, if you go to the github dev branch you can see the game is being worked almost everyday and the last updates are mostly small fixes. I imagine the 1.8 is coming soon. It will focus on transformation for what I could see but maybe they update the random encounters or the "look for trouble button" as well.
Judging by the dev branch on github, it looks like the update might drop very soon. Latest work has been minor fixes and implementations. Looks like its gonna be an absolutely massive patch too

That's not the random encounter I was talking about, I already know about the random things that can happen when you walk on a square that someone else was on based on where I am. I'm talking about the Pokémon type of random encounter for when you step on an empty space something will have a chance of happening, and every time I pressed the "look for trouble" button it tells me that I can't do it in the location that I'm in. I'm in the cafeteria/by the gym when I hit it too.

Continue Tavi's arc? Yeah, you need to get her corruption to either 200% or 0%. To max it, make her climax during first meeting of the day, IIRC some other interactions help as well, but don't remember which. To zero it, deny her orgasms. Keep doing so and eventually you'll hit one path or the other. Keep in mind that there are milestones at 25%, 50% and 75% (both on corrupt and pure paths) and your progress might get decreased a bit, don't get discouraged, it's fine.
I don't remember seeing those around, like random ones. There are a few events that trigger on specific places like the gym or the greenhouse when you interact there. Some events trigger around when you are doing quests.
During the update that allowed the NPCs to move around the "look for trouble" button was disabled, because you can actually find the NPC on the map and interact directly with them with the "look around" button.

There is a mode called Brutal dreams correctional center that make more combat encounters.(Can turn this game into well... Think pre year 2000 RPG amount of fighting)
About Look for trouble in-game thing. It now depends on your population and where they are.
Since alive NPC thing was recent, i would say wait for a new patch

At some point you will meet an inmate called Vion at the liliac block, the bottom one. He will give you hypnosis sessions that give xp for that skill. I personally didn't dive to much on the hypnosis stuff, but I imagine you also gets xp when you use hypnosis on the npc and when they use it on you.
testing some stuff in nightly build. Seems TF stuff is mostly complete. Also collecting fluids now have a purpose. You learn use them to crate TF stuff. And new scenes with Eliza. Also new bounty board in near Nurse. Do stuff here and get credits and science.
Also we now have dungeons that we need to delve. And it seems your level reseted in here to first level. Also you go here nude with empty inventory.
And TF stuff not instant, each TF item do it over time. Also you can reverse transformation or fully accept it. Your character only have one "backup" DNA slot

First time playing the game while having no idea what I'm doing. Found a guy selling sex, paid for it, suddenly demanded more than I had, and got into a fight with him.

That can happen. Also there isn't a right way to play the game, there some story lines you can follow but you can do whatever you want. The only limitations are the game mechanics but the dev is constantly expanding them.
