You can get enlarged breasts and those are permanent, however CGs don't show multiple breast sizes, only the bust of the protagonist does (due to budget limitations, mostly). Same deal for tattoos and piercings (piercings might show up in a handful of scenes if they directly relate to one, but it's extremely rare).

Hair color changes during one event, but it's not a permanent change.
