Are you paying attention to the girl's Pressure bar? Not the Tolerance meter, the Pressure bar? If that fills up too much, nothing else matters; they'll leave. If the bar is well over a line and says "Vulnerable", you need to lay off.

Did you by chance use the in-game cheats, cumputer->Settings->Modify Settings to alter the girls arousal gains etc?
If so, try setting them back to default, hover over will inform what HeX recommend as default.
If You didn't meddle with this, then know that when a girl orgasm her Tollerance takes a big notch and can cause her to leave, MC cumming also affect her Tollerance, so keep an eye on this stat of hers during exams.. if she has low bar left of this and tries to get your attention, either ignore her, or go back to your desk after approaching her, to avoid affecting her Tollerance.
Their Tollerance goes up if you focus on another, Give a hint, or Incourage the girls.
If they have a buttplug Tollerance decrease with every action, including what would normally Increase it.
Max Tollerance is something that increase over time, especially if they have Athletic Trait and is capped at 225 I believe it is, but it starts out way lower, like 160 or something, depending on their traits.
School rules, whats allowed and not, also affect their Tollerance, so traits that considder certain actions to be allowed will not affect her Tollerance as much, even if you haven't passed the rules regarding them yet.

Fixed the bug. I also moved the massage videos to their own mod so that you'll only have to download the vids once, and if I update the mod then the update will be quick.
This works for all of the girls in the current exam

Just for basic troubleshooting... How is your hard drive space, do you have any download limits through your ISP? those tend to be the usual culprits
