where the file for update 0.46 its been month since new update while this version stuck at version 0.43
After beating the orc in The Bog, you can go into the prison (go to the goblin throne room then the right passage) at the end of the prison area is the orc in a cell where you can fight her again. Beating her might give you the axe.
Does anyone have a clue on how to get to that secret room in tower. Ive played the game to its fullest and Im loving it but this one secret room in the tower is eating at me
The Key is located in one of the chests in the same room where you confront the Prime Poltergeist.

Easy there gringo, try to disable cruise control first.
I will need more info in order to try to trigger it and find the issue. Which teleporter are you using? Where are you teleporting from? What ending did you choose on the Goblin mission? Any thing else that might had trigger it?
PS. There is a chance this is the same bug as the faucet bug, let me know if you used the faucet in the house, if so, this bug has been fixed, you'll just need a newer version.
