Bathing is already a mechanic and soft requirement to lower your allure, and eating/bathroom usage are nearly universally disliked in game design for being annoying and tedious.
Pregnancy has a shot for the fetish side of things, but I dont think those do...
Yeah, but I'm just saying, "just saying" if the game is about "surviving" in a city where literally 98.9% of the people want to rape you or are only a hair's breadth away from doing so, you're an orphan who has to pay a debt that he/she never wanted or else they sell you as if it were a ham and you have an obligatory commitment to have to attend school or else it generates crime.
"I'm just saying" yes... at the beginning of the game it's fucked up and difficult if for example you seek not to lose any of your virginities or you want to "stay sane" (because if you play not to care about anything, then the game becomes much easier) . But after passing the first 4 months (and still without a guide) the game becomes extremely easy, I mean... either with the brothel or honest work, at that point you get a stable source of income (and with honest possibility to increase even more) which lacking the game of necessities or things to "spend" once you can start saving, the main element of the game that is the payment of the debt of the orphanage becomes nothing and In my opinion the game loses meaning from that point.
That is why I say that even if it were tedious (for both parties), a solution, even if it makes the beginning more difficult, would be to add elements such as "hungry", "health", "thirst" and "bathroom needs", since with this example, it would prevent you from being able to work completely by having to go to the bathroom or have to eat in the middle of work (which, and taking into account how the game is, would give to create more events such as having to go to the bathroom but depending on the place you can get to assault for money or for service) which would mean that instead of you having dominated the game in 4 months, between including the times of eating or going to the bathroom (necessities or cleaning), health (after confrontations if you fight or even if you agree to meet their demands) plus the existing base (debt from the orphanage, sleep and the need to attend school) that would mean that now you are more pressured, one if you were not careful or if something went wrong you have out of necessity to do more dangerous things (brothel or even robbery ) which now would make sense that due to bad luck or bad action, you would fall in jail or in the asylum or even in the underground brothel, which in the case of the last point, would make sense that fulfilling only basic needs ( school and payment for the orphanage) because of the danger in the area, one would have to go to the farms to be able to work and continue generating money until things calm down in the city.
Short summary:
