well, 19.0 has been the last major content update and was about a year ago. all "updates" since then have been bug fixes and translations. Before the steam release all updates brought at least a bit of new content. but it looks like the next update will finally continue the story again.

Yeah man, i had the same feeling, the sandbox part was mostly like a full on game, go wherever you want and make the choices you want, follow routes at all, i still feel like we have few more years of desert stalker, maybe another 5 who knows?, anyways im excited for the future.

Because of censorship but it's like that for a long time, since the earlier versions of the game. You can change the relationships in the options.
