Ok, I'll explain my case of how i got the game working (at least in my case, also i have a Japanese region without apps).
First, I downloaded the game and moved it to another hard drive inside some hidden folders.

Second, I unzipped it with 7zip, and when I double-clicked it, the game wouldn't open, but it appeared in the task manager.

Third, after many attempts, I remembered that some programs don't work if they're not on the OS disk, so I unzipped the game again into a folder on the hard drive that contains the OS, and IT WORKED!

I hope this could help some of you

To be more specific it says in the tutorial that you can only save after the body check screen appears at the end of each day/highlight. You cannot save at any other time.

You need to run the game with a Japanese locale. Search this thread for "locale" to get instructions.

In the future, you can know this is your problem if both
1) The game seems like some Japanese shih
2) You see a bunch of ??? or �� or ʸ»ú²½¤±

Unfortunately the new patch seems add nothing in-game. Only new images in the gallery once finished the game. Images seen online way earlier.

The devs moved onto other things, tokyo is one of the bigger h-artists out there in doujin scene and has had a few other games he's had commissioned at this point. Any further patches are solely localization improvements and bug fixes.
