There's only so much you van do. Takes 9 months for a baby to be born, during which you're not anywhere near as capable, and caring for one takes years, unless you're mad/desperate enough to bring the child off on adventures with you. It very much puts an end to a travelling, adventuring lifestyle for too long.

it's literally the fantasy world and it's Elf we are talking about here, they could just use magic/mutation/bullshit seperate rules/ whatever means to justify it.
there are no realistic aspect in those kind of game so does some rules that can be changed in said world.
in H world, there is zero reason why some fetish couldn't be done and being "realistic" isn't one of them.
unless it's life simulator, kick ass pregnant mom stride around like a fucking tank is fun!!!
