It's a new experimental feature in the latest Patreon build. If you're on an old save it should pop up after loading, and if you're starting a new game you'll be given the choice right before meeting them.

I've released a small followup update to add the missing population control option. As for a proper big update, it will probably still be a few weeks at least. There is a ton of new features that I have to flesh out and then playtest, before I can release a public version.
As of EA build v1.28.5:
Bugs
- Ealga assets still missing during various service scenes. Instances occurred when in anal missionary. Her hair was missing. Turning her around had her model just vanish. Vaginal has doggy work fine, but missionary has her face just not show up.
Just tried the latest version now so haven't found too much other than that yet.
I'll update if/when I find more bugs.

Am I supposed to be able to do quests in version 1.28.02? I have 5 Main quests that cannot be completed and 1 optional. I don't know if it's a bug or just not implemented in this version yet. Basically after finding the chest in the sarcophagus and having the meeting I can't do anything else. The locations all just say "Coming Soon" or have no special interactions, and I can't tell Blanche about the letter.

Most of them are not yet implemented. You should be able to do most optional quests that show up, but most of the stuff you see just isn't in the game yet.
As of EA v1.28.6
Bugs.
- New doggy style interactions with thicc body types trigger a number of error messages that require a 'rollback'
- Battles during quests are still pretty buggy. Namely, same old issues of PC hitting an enemy and adding to their health pool on the fist hit, dragging out the fight, party members not displaying/having a turn, inability to have sex with party members, and... a new graphical error.

I haven't run into any other bugs yet, but a few of the ones I have found have stalled play sessions. I'll update if/when I find any more.

I found out that if you want a fast progression in relations with any NPC, just press the "talk" button, and there will be a "gossips". You need to hold ctrl button and fast click the "gossips" button with a left mouse button and that's it!

Alright, thank you for notifying me and providing the logs. I was really hoping the quest bugs were resolved after the second to last update, but it seems I'm going to have to dig a little further. As for the new position, I've already started to make fixes to it, so it shouldn't take too long.
I'm sorry to hear the bugs have affected your play sessions. Hopefully, I'll be able to do something about them soon!
It's not unexpected.
I mean, these are full on RPG elements being made for an engine not really meant to handle them.
Bugs like this are to be expected.
I mean, the questing is really cool, but I'm not expecting it to be easy to implement.

- I know this is prolly gonna annoy the crap out of you... but, anal with thicc body type girls still gives you an error message.
- Ealga's anal option won't lead to an error message, but her assets vanish.
Those were the ones I noticed so far.

That part's slow going though but oh well.

Alright, thank you for sharing these errors! I think I've finally got the culprit now, so it shouldn't be a problem in the next update.
I have a curiosity, is there plans to implement a sort of autonomy system to the NPCs have sex with each other similar to "influencing" game by golden crow? and even letting to also share your girlfriend/wife with other people? it don't even need to be something like NTR but like allowing your girlfriend to do it

I do want to introduce some sort of a hooking up mechanic for romanced characters, but there is still a lot more other bits of content that I have to take care of first. At the moment, there is a prelude to this in the form of threesome scenes. I plan to have it so that doing threesome scenes with NPCs will make them get acquainted and potentially hook up together if the player encourages it.
