The instance you mention, a shoe-fetish, would be a legitimate fetish, and like you said, someone with that fetish would never get the same level of excitement without their fetish, but that doesn't mean they need it. Like I said with my explanation, the idea of fucking in public is exciting to me, it gives me a greater feeling than pretty much any vanilla sex, however I don't need it to get hard or orgasm.
