you have to attempt to train kora where you train with the spinners(I believe her stats have to be as low as possible at that point in order for the "train with kora" to pop up). It'll say you need a new scroll. This will unlock the option to buy the key from shady which opens the door on the statue island, giving you the scroll.

You are right, 4 endings. But there is also a foursome or threesome scene before the ending.
The endings can be triggered by altering the "apublic" and "morality" variables in the console.

You get the foursome if you have told mai where her weapons are, if not you get the lesb threesome.

No and no.

You get the mask if you follow the main quest. If you're stuck, try doing the spinner game until you're at least level 8 and then go to your room. (And of course use the latest version 0.8.2b)
