Theorically it would fall under trasgender if he* wanted to undergo surgery, but if you aren't willing to it's crossdressing and a good one.
So I would go with crossdressing or trap (we could even use the sissy word if he has a tiny penis). About it I don't know your game, but depending on the theme it would be well received or be a mistake.
My recommendation? ask the people that play it if they are fine with it and decide upon that, unless you feel it's a necessary part of the plot or something.

Your character would be transgender not futa and as they have always seen themselves as a girl would more likely prefer being on the receiving end.

Jumping in blind and responding to OP without reading thread yet, and prefeacing with that I don't really have strong opinions on Futa characters (a kink that doesn't do a ton for me, but is a fine 'occasional' thing), and Herto male. I will say for whatever reason, I see a LOT of futa pictures around, weather there's simply a MASSIVE demand for it in particular, or if it's seen more as having broader appeal (partiuclarly the VERY busy, VERY feminine futa with MASSIVE cocks).

There's no need for it to be pre-puberty. In fact, but I don't guaranty it, I more or less remember of a talk about this with a doctor, saying that it can in fact be a real problem to do the hormonal process before the end of the puberty. When a transgender (because your character is a transgender) pass through the hormonal therapy, (s)he live a second puberty. I assume that what the doctor was saying at this time was that it would have needed to double the dose, since at this time the organism was already boosting the hormonal production through the natural puberty process.
This said, for the surgery, it's not mandatory. Not all transgenders transition from their birth body to the body they should have had. And not all the ones that transition, go through the whole process. The main "visual factor" is in fact the boobs, that what people use to differential a girl and a boy, and so the body part that is the most usually removed/added (depending in the case, in yours it's "added", but not necessarily by surgery, the hormones can do a great job by themselves).
So, you can simply say that your character don't felt ready yet to be ride of his penis, or just ceased to feel a body dysphoria once she grew boobs. It happen.
But, there's a thing that happen in the majority of the case (but not always), it's the need to remove the sexual organs by themselves. I have few return coming from MtF (Male to Female) transgenders, but I heard way too many FtM ones talk about how it was really painful to keep their ovaries once the hormonal process really started to act. To over simplify it, ovaries and a really high level of testosterone aren't really compatible, and it's like the body tried to destroy this part that shouldn't exist. So, your character will probably want to get ride of her testicles, or discover what women live once by month, but for her it will be more often than this.
But, again, this is also not something mandatory. It seem to happen in the majority of the case, but not always, and some transgender live it better that other ; probably because the level of pain isn't itself a constancy and it's more bearable for some than for others.
What you describe is a MtF transgender. But you can also explain the absence of body dysphoria by the fact that she discovered "dickgirls" by reading hentai, and felt that it is what she really is ; neither the boy she was born as, nor the girl she firstly thought she effectively was, but really a girl with a dick. So, while being technically a transgender, she can identify herself as dickgirl without problem.
Not necessarily. She's a girl, and there's absolutely nothing gay for a girl to take it in the ass. It's her partner that need to be bisexual or pansexual (a gay wouldn't be appealed by the boobs and the girl looking body ; and will not support long enough the girl personality), for her it's a straight sexuality.
But if you plan to make her have sex also with girls, then she must be bisexual, but will probably define herself as pansexual. In fact, it can be this pansexuality that lead her to the dickgirl option. She always felt appealed by both male and female, and so felt it more natural to stay this way.

That said, I think, developers can increase their numbers greatly, by replacing futa penises with strapon dildos and giving player the option to choose on of the two. It would turn a futa scene into a lesbian strapon scene and make your game attractive to exactly those guys (and much rarer girls) who don't want dicks in their porn.

Replacing a futa with that requires a lot of work, current games don't have enough resources to let players choose things like that.
Even more I have to say that trying to "work things out" don't appeal to anyone. As most things it's have or not have.

Wouldn't it just require changing the color, so it doesn't fit the character's skin color any more? I'm certainly no expert on this, but changing a piece of meat into a piece of plastic doesn't sound so difficult.
Normally it's quite a lot harder than that, you'd have to incorporate the straps being that hardest part, else you'd notice where it joined and this is normally quite a significant area of the crotch, not just the penis itself. If they're using Futaliscious you'd also have the problem of the vagina being far too far back as it'd normally be more where the dick is, and if you just retextured the penis you'd give them a plastic vagina as well. The only way I can see a change being done would be to render it once with a dick, then spot render it retextured to plastic, and then maybe add straps in photoshop?
Otherwise a more practical way would be you'd need to render the scene twice, once with a strap on, once with a dick, and you'd have problems of the dick and the strap on not being the same size, meaning you'd have to make posing adjustments with the vagina or whatever they had the penis in each time. It would turn a say 20 render scene into a 40 render scene with adjustments between the two sets, and that over the course of a game will add up to hundreds of different renders. At that point the developer would probably think is the appeal for strap-on sex instead of futa bigger than the appeal of another hundred or so renders in the game, and the majority of players would probably prefer the time was spent on extra renders.

Hermafrodite = a real person who has primary and secondary sexual characteristics from both sexes, in various ways. Some may have a functioning penis and vagina but some may have an underdeveloped penis or vagina which are not suitable for sex. I don't know any hermafrodite personally but I can imagine it's pretty difficult for them growing up. And if their sexual organs don't work properly it's pretty hard in adulthood as well.

I do know some of those IRL and they are really interesting people generally, who have made a fundamental decision in their life (which is something a lot of people never do!) although they do struggle with it, even if they say they don't. From a story point of view I really like this kind of struggling, (although IRL it's not fun at all) and I would actually really like to see a game about a guy who actively decides to transform into a woman. I don't think it exists so far...

And finally, is any of this gay? Who cares... Maybe it'll make some guys get over their fear of cocks which are not their own. I always find it passing strange that 99% of the futanari I see in stories or being RPed or what have you, are always very dominant feminine identifying, and almost exclusively consider themselves lesbian. You rarely ever seem to find Male on Futa or even Male on mtF or even 'Traps' for that matter. Not rightly sure the reasoning behind it, but just couldn't help but notice it, particularly if you're ever on forums or in something like WoW on Moon Guard. That said, I agree that the two terms are becoming more and more synonymous, though true hermaphroditic characters seem to be becoming more rare in favor of those with just a (normally significantly oversized) penis.
