But my question is, do Futanari exist IRL in Japan (outside manga / hentai) or are they purely fantasy?

Intersex / hermaphrodites (i.e. persons born with male and female genetalia) obviously exist, but are rare and can take many forms, including some pretty strange ones apparently.

I am not sure if an actual intersex person would be called Futanari in Japan or something else.

The variations of Futanari is not a complicated subject.
The two main ones are -

1. Dick + Balls + Vagina (The best iteration)
2. Dick + No Balls + Vagina
