But my question is, do Futanari exist IRL in Japan (outside manga / hentai) or are they purely fantasy?

Sex is the scientific term used for denoting the difference between male and female
When it comes to the human species anything that has as Chromosome of Y in it regardless the number of Y or X chromosomes is Male. Any that do not have a Y regardless the number of X are all female.
