I wager you have more artistic freedom when using an oversized dick, at least when your audience are primarily men.

Since you can't convey technique or feelings like pain and lust easily, doing it visually by exaggerating the penis size also gives your protagonist motivation and/or a way to gloss over character flaws that are popular in the ntr/corruption genre.

Games are a visual medium and although the dicks may be comically large by real life standards the scenes aren't happening from our perspective, a large dick makes it more graphic. Another thing, women lusting after a large cock like crack maybe unrealistic, you need to ask yourself what's the alternative? In real life the men who have harems are almost always extremely wealthy (or cult leaders, there's an idea) and although realistic, women lusting after the MC because he's rich doesn't have much appeal.
