There should be nothing wrong with contraception but perhaps the shot covered two ovulations and you are about to become pregnant just now or else you've just been unlucky(or lucky?). If you don't mind spoilers you can just check the cheat menu index's variable tracker.It's been a while since I checked this, so it may have changed, but from what I can remember...
Basically, the Volleyball Coach will never deliberately cum inside the V, nor is there any visual signs that he has. But, you can go into the cheat menu and click on 'OOX Debug' and you will occasionally see that he has deposited an amount. I assume it is because he tries to pull out but just miss times it a bit, or something like that.

Is there a way to make appearance always 10 out of 10? Glamour spell used to do it but now it's only temporarily.
That's odd. What do you get if you console
tabletkishot and birth_control['implant_status']? 40k pillcon means you are perfectly covered by some form of birth control.If you are not playing playing with bimbo character you could set
extra_supnatvnesh=100 in console to increase your looks by 100. Bimbo level 2+ will overwrite it but otherwise it should remain indefinitely. Change the number to increase or decrease the bonus.

You got 44 days left of birthcontrol shot. If you want to skip ahead you can do
tabletkishot=1 in console and then it should end the next day. =0 might also work but I think it's safer to let the game realize it ended just to be safe.

Still something actually happened. The kidnapping event I did in back like July, in full. Now is December lol.
And apart from the therapist pills I can't think of anything else to have been able to mess with that birth control shot.
The abduction gives you 84-91 shot, same as getting it from the clinic. Those are the only two occurrences of this variable in the game. I don't see any possibility where it might get extended but if you experience it returning or extending again try to keep the dates and situations in mind so it could help narrow down where and why it happens.

The character's sister advises her to try focusing on blowjobs and anal sex in order to keep her virginity, but the character has almost no such option.
Most of the scenes don't have the option to choose anal over classic sex and that's sad.
I understand why this is normal for story characters and events, but I don't really understand why the anal option is missing at hookups and bootycalls and other generated events and characters...
I hope someday these options will be added, and the character will be able to follow the advice of her sister.

I narrowed down the issue with blank screen when interacting with Katja and visibly pregnant.

This is starting to get on my nerves:
When you suggested to
tabletkishot=1, I did just that. TV'd 2 days and checked tabletkishot which indeed turned to 0.Now I tried just to wait those 44, never goes down. Waited until ovulation, still stuck at 44. Then tried with 1 again from same save. Still stuck at 1 lol. The hell?
Good catch. Seems like we're not checking if you actually are on the birth control shot, and the only way the counter goes down is if you are flagged as being on it. In your save she thinks she's on the pill so the shot counter doesn't get reduced. In other words going on the pill while being on the shot makes the shot last infinitely.
And as a personal quick fix out of the problem you could probably survive by setting the pillcon = 0 and tabletkishot = 0 in console separately. But as always the console cheating might also cause problems so at least a backup save is the smart thing to do.

Katja pregnancy content were disabled though, in the files u can find:
!!Out commented Katja becoming pregnant until more content for a pregnant Katja is done
My bug was about PC being pregnant. Katja's reaction about you being pregnant caused blank screen. Unless that save provided in my comment doesn't cause blank screen for anyone else. If it doesn't then it's just my player that refuses to accept it. Not entirely impossible, it might be the Linux client works different ways than the recommended Windows one.
