MB has told us repeatedly over the years that there's no twist, no conspiracy, she really is dead. It would be a shitty thing to do, to go back on it now. there's really no need for it in the story.

You're correct, it does happen a lot. But this would require multiple people to have amnesia.
Grandma, Amy, Liz, Kat & MC would all need it, as well as his sister.
Even if she got it from the crash, we know that the other 5 aren't suffering with it. As clearly evidenced by them all recognising each other.
You can't just give it to one person as explanation for why she doesn't know them, then expect 5 others who aren't suffering with the same condition to also lose their memories of her.
Seems a bit of a stretch...

Even in a game where the MC uses a variety of inappropriate tools to fix cars, logic still applies.

That's quite easy to explain without other people having amnesia:
1. At the start of the game it's been 6 years since the accident. Especially when someone gave themselves a completely different style it's easy not to recognize them (for example if you went from vanilla to complete emo) after such a long time.
2. They think the sister is dead, so even if someone comes around that looks like her, but uses a different name and doesn't recognize them, it's quite easy to accept that she's not the sister.
3. She is someone that during the time of the game so far only Thomas has met and none of the other family members.
