Go above the orc cave and then to the right. If you go into the cave, you just visit the merchant girl.

Here, just run the recent autosave. Everything should be unlocked up to this point. You can teleport to Vivi's clinic basement directly.
If you don't know what and where still, I can't really help you because you're not trying hard enough, have a fucked up game or I'd have to write an entire walkthrough.

Got it. The Unity Translator works like a wonder.
It takes a few seconds before translation takes place so you need to wait for a few seconds.
