Bare Witness was quite frankly one of the best porn games i have ever played. If this will be around the same level then it will be a great game. And with true harem this definitely have a potential to be even better than BW.

It's an alternate universe (the devs studio is AlterWorlds). So the Nikki here is not the same Nicole as in Bare Witness.
I'm all for representation, but would like to point out that only about an estimated 5-12% of trans women get genital reassignment surgery. So it's far from unrealistic to include trans women that haven't undergone it. I do agree it would be interesting to see a trans woman in an AVN that has gone through the surgery though and really only comes up through dialogue.
Since people have been taking about it, I'd also like to mention that the devs have said that none of the main LIs are trans. I virtually guarantee that when it comes to the optional nature, the player will not choose if the character is trans, just how MC interacts with them.

I can picture a scenario in my head where MC dies but leaves the girls pregnant and with his wealth and it's a bittersweet ending sort of deal and I wouldn't be too mad about it. Hopefully he recovers though, or becomes a cyborg.

I can agree with this and attest to it. Twice over the last 10 years my father has been on trial drugs and had fantastic success with both (a rare leukemia). The current one which he started 6 months ago is literally revolutionary.
Perfectly acceptable/believable plot line for me.

or he could just die at the end, not every story needs to have a good ending, but that's just my opinion.

there is no way someone can't spin the NTR angle no matter what happens in this game, it's a strip club, so comes with the setting.
I agree. Just saying that if they want there to be a happy ending, there's a path there that isn't too unbelievable. So I wouldn't write anything out yet.
It also could be left open-ended, where the cancer isn't cured, but not actively progressing due to treatment, and the epilogue is them deciding to make every day count out of the uncertain time left or something.
