With both Sisterly Lust and Summertime Saga you can get practically everyone pregnant. It doesn't change the main story but it will affect the models and dialogue. Both are fantastic games. Edit: I should clarify that with Summertime Saga you can't have sex with any of the women while they are pregnant unfortunately. With Sisterly Lust it's very much the opposite.

Suggestions for brother/sister focused action? I played Milf City, Midnight Paraside, Big Brother and Dreams of Desire.

Sisterly Lust and My New Family both have 3 sisters (and other women, optional) to romance. If you want something a bit more focused on B/S, check out Haley's Story or Summer Scent.
