I would advise you not to judge a book by its cover. you'll be missing out on a lot of Great VNs if you are focusing on just the art style.
for example:-
My New Family, at first sight, looks like a generic-looking VN made by Honey Select. and I still remember when Light of My Life first came out, users mocked it for its unique art style.
and yet these 2 games are now considered some of the best on this site thanks to having interesting stories and/or beloved characters.

On Steam version MC is adopted, and I know of no patch to fix this yet.

I absolutely agree with this. LomL's art style threw me off at first, but now I find it wonderfully unique and I feel like it adds to the story in ways I can't put into words. (EDIT: FYI, if you like this style, Making Memories gives me the same vibes.)
Art can be a positive or a negative, but it shouldn't be the only criteria... with some exceptions.

Great piece of advice! I have misjudged a fair share of games like that. Something I found out only recently, searching for titles similar to a heavily story-driven game a friend of mine just finished. Although checking out a few hundred games in the past decade, I knew none of the titles I came across. Turned out I had selected the majority of games by the attractiveness of the characters in the preview images. Missed-out on the titles you mentioned as well, I will give both a chance this week.thanks for the kind words

Any similar games like Measuring my Cum or Power Vacuum where the Mom is helping the son out with some sort medical situation? Don't want it massively romance focused but mainly just the Mom trying to be helpful if that makes sense? Preferably it has M/S but its fine if it doesn't.

Prince of Suburbia comes to mind, though it has a definite breeding focus, and it doesn't take long for the son to "heal".
Also Lust Epidemic, though I think with the patch she is the Aunt and not the Mum, though in the sequel Treasure of Nadia, the patched relation is definitely Mum.

Yeah... I don't know what to tell you. If you are seeing not related/ adopted something is going wrong on your end, it worked fine for me. The patch is the one right on the first post of that thread clearly labeled as such. it's a mega link. Whelp it appears it's back to adopted. In 0.4 the dev had removed the adopted references leaving only one reference in the skippable intro video. He even posted as such on F95. But after finally getting around to look at the 0.5 script it looks like he went ahead and put a new adopted reference into the game in the optional nun scenes.

You define the actual relationships at the start of the game (the "defaults" are stuff like tenant/roommate/landlady, so it's up to you), and there's a slight possibility that the "M/D" and/or "S/S" tags might be in the future. Who knows?

I find myself curious, I've played a fair number of incest games and most of them appear to hesitate in regards to fully leaning into the actual taboo of incest.
Oh sex is had and the info dumps at the start stated they were related but; and this is especially the case with works funded on patreon who had to skirt the rules, you could easily just assume the characters are friends or roommates and not be required to change much about the game.
So, does anyone have a work where the actual taboo, the immorality of what the characters are doing is important and/or explored?

I fully understand your pain, it's the same for me. I'm only into incest because it is the "last great taboo" that can be committed consensually. I really dislike the current state of incest games, where incest is basically the default and not something you have to hide at all cost. Games like My New Family don't feel like incest games. They're just harem games.
The worst offender I found is Lust 'n Farm. That game is utter garbage when it comes to incest. It's so terrible that, despite knowing it's not on the list, I won't report it. And I report really shitty games. It simply does not belong on a list about incest games.
There is only one game I can think of where the consequences are explored - 2Sides
Then there's Refuge of Embers which talks about consequences. But the game's story is barely out of the "prologue".
