I think it happens when you switch between any actions too quickly. You can just pick another action to do that is not major and it should make the game "realize" what's actually going on

Didn't know they existed because I didn't browse through the assets, but I know after you unlocked the What is Happiness? node (I forgot the real name it's just the last node for this phase), on the right of the Life Lesson screen, there will be a bubble that says アップデートこちらから (appudeeto kochira kara). Click on it and then some new nodes after the final node will appear. Maybe some extra contents are unlocked there.
