No they don't. If I'm correct, Japanese artists send their uncensored H-Mangas as pdf files and they get converted as HQ Pngs images.
I suppose laziness may be a reason.
It is difficult to draw a good looking guy. It takes much less effort to draw a bad looking guy. Spending little effort on the guy allows the artists to spend more effort on making the girl look good, which is more important for most of the audience.
