I dont understand why netorare has to be someones which has relationship (wife/girlfriend) with you/main character. No IT DOESNT. Netorare its all about broken heart, or despair or something like that. it can be achieve by betrayal, or even something that main character can't get into (in romantic or sexual way). It can be main character wife, lover, love interest, crush or even someones close to them. Crush or romantic interest even usually has more ntr tag in japanese doujin/games.

Relationship does not defining netorare. just be broken heart.
