The choices you make drive the MC to the dark side (psycho) or the light side (sane). It's your interaction with the MC, through his thoughts, that drive the choices he makes.
Ultimately, this will create different paths the MC will follow, and disable/enable options for him to do.
For example, if you're always making nice choices, driving him towards the sane side, and there is an occasion to do something psycho, you will not be able to do it, because you're not psycho enough to do it. The same goes the other way around.
