My preference is realism because real life isn't only sex. You have to survive in between the sex. I sincerely attempt to comprehend the story in between scenes to maximize immersion
Well, a lot of porn games aren't realistic or attempt to mimic real life 100%.

Here's a quote from someone else for his game in the making;

1) Lewd cut-ins

Basically what this means is having cut-ins for things that aren't in the actual artwork, but are being described in the scene. A good example is, "You make out with x" and while I don't have the time to illustrate every character making out, what I could do is just make a small cut-in with lips and tongue. Then recolor the skin and lips to match the character. If you've ever played Persona, it's like when the character's eyes cut in when you summon a persona. I had started working on it before getting a little skeptical about it. So here's some linework of what it would be, quickly put onto a screenshot from the game.

From all the porn games I've played, this is everything I can think of:
  • The previously-mentioned lewd cut ins
  • Sex animations
  • Nudity (though it's not very impactful)
  • Lewd character status display, seen often in text-based games like in DoL
  • Modular sex animations (usually)
  • Small sexual imagery, like the card art in porn deckbuilders
I don't think it's really important what intermediate content is in terms of it's presentation, all it needs to be is impactful and minimize it's negative effects on gameplay.
