He instituted a new thing where with each update of the game you have to put in the new extreme code or it will disable all extreme content. Even when loading a save.

I was just stoked to see someone acknowledging the desperation required to be as invested in this game as many commenters are.

Patch for 1.20 that enables extreme content.
Unzip and paste into data folder.
No need to enter any codes at sex maiden, but if using an existing save you might need to sleep once for it to take effect.
Cheat codes are the same as always. Relationship cheat code also works (5010).
If someone knows a quick way to get to the DLC, let me know and i'll see if it can enable that as well.
