The file 'patch.rpy' is literally an empty file. Just open notepad, 'save as' the blank file as 'patch.rpy' in the game's 'game' folder and restart.
Should do the trick... assuming you are trying to get the Incest patch...

Nope, they just added animations to existing scenes.

Depends on the game. This game just looks to see if the file is there to activate the 'choice', other games use other methods like changing the game text or character titles in the file itself. This one is simple, which is nice.
