You have somehow spawned your offspring on the tiles reserved for the Submission war quest.
There are 4 color coded sections reserved for each castle region.
Those tiles spawn imp swarms customized to each keeps fetishes.
Clear the red castle and those tiles "should" return to normal grey.
The red tiles belong the Tomboy Streetthug Dom faction IIRC.

Ok I haven't played this game because I was recommended not to play since the content was very small and there was some drama regarding the Game creator being lazy.
It has been many years after that ordeal, is there enough content to play now or will I have to check this game after a few years again?

Development is indeed slow and the developer never delivers on their promises on time, presumably because they work best when over deadline, since they have been setting and missing deadlines since before the development blog has even been public.
We've had a slow but steady development on this game for 7 years now, so I'd say there's at least a few days of content to play through. You'll have a good idea if you'll like the game after doing a few random encounters and messing around with the transformation and fetish system a bit.

I'm... gonna assume you're talking about double penetration. So if you're not... that's on you for not being clear.
First, you should re-enable companions, at the bottom of the Content Options -> Gameplay option list.
This isn't strictly necessary but it makes it easier, as the only other ways I can think of to get yourself double-stuffed like an Oreo are random encounters with multiple enemies, like the imp gangs in the tunnels or Enforcer patrols. There might be a few more scattered around, but I wouldn't know where.
Then, you need to be able to position people manually, which kind of gets in the way of what you're going for because unless you have the trait Irresistible Appeals, you'll likely be ignored since you're the submissive party.
Finally, don't look specifically for a 'DP' option under sex actions. There is none. You have to position yourself between the two other people in the scene on all-fours or on your back, or ask them for that pose because you're the bottom, and get one to take your mouth and the other your ass.
Just top from the bottom. Seriously. It's easier.
It's getting easier to type these things, but the realization still hits like a truck.
