Basically after you reach the end of the first adventure (The one with 100 levels) you get like a little cutscene where you and your sister talks with some person at a giant tree, it is possible you missed a thing the person says and i don't remember the exact words but basically they ask if you're willing to give up your ability to impregnate women in order to halt/slow down your sisters illness to which the brother doesn't even hesitate for a second and agrees to it. So as long as you've completed that you're safe to creampie all day every day x3 Enjoy~

No reason why not. I've been meaning to finish a 'canon' playthrough if the sister travel translation is to be believed.
The game specifically prevents any events (including random ones) outside of "night activities" from taking her virginity; even if you ask to do it in the bath, she'll refuse, saying that she doesn't want her first time to be there.
