Depends if you are taking the love or lewd route.
Lewd: You need to buy the higher level outfits available, then unlock them with a reward point (when you are eating with her). The clothing store should say something along the lines of "you could convince her to wear this" - thats the one you need
Love: Whatever love stage you are at should have the appropriate outfits available in the store, just buy and she'll wear them herself

Both need you to do the clothing events that appear once she is wearing an outfit, usually in the kitchen at breakfast, and when she is doing yoga. In order to do the actual leveling up. There's an explanation in the help/tutorial section on the computer with some of the icons and such you need to look out for.

Jen yes as part of her stripping intro. And I'm not sure what you mean by the modelling on the weekends. She has a work schedule based on what stage you are at and where she is working. In fact I think at the latest point the modelling job is done at the weekends.

After you unlock her home she switches over to a schedule, and will only appear in the gym at 16:00 I believe. So try that, and if you still have troubles post your save file for me and I'll take a look at getting it fixed.

I second this. After I see Lydia for the first time, the gym only shows me 2 options: Head Home or Stay in Town. I have not actually spoken to Lydia or unlocked anything from her. It just happened after I saw Lydia the first time.
