0.18 was under beta for a while in f95, update release 0.18 should have the custom raid level totem, if it doesn't then it wasn't updated yet.
CDDA is far more brutal than rimworld since both use descriptive stats to "explain" the gore, also caves of qud is similar to CDDA whereas if you get something brutally chopped off the game will tell you.
graphically it's on a similar level to conan exiles as you can also dismember people in that game.

Gore... is not really such a big deal. You can dismember your enemies even in mainstream games like Fallout 3+. That is certainly not something that makes Mad Island special. Making shields out of your enemies' hands does make Mad Island special.
So no ending in October but a cave dungeon probably like the ruins, the lab or the prison. There at least better be something to fuck and bring home in this cave, and i don't want any mummy blow/handjobs neither. I honestly don't mind this cave dungeon, in fact i welcome more. Let's hit that zelda standard of 8 and let's have something worthwhile in them, both mechanically and erotically. Maybe Nami could be in this cave finally? But this would be optional content so maybe not.

the gore table is kind of unique to the game but it's on par of the developers stuff, they also ported the other two characters from their other games as is, nicky has most of the little life brutality animations if you look it in the animation gallery.
they are missing that bottom bitch which becomes your character's love interest from black market.

RJW is essentially a framework. Of course there's not much to see if you just slap it on top of vanilla without any other submods. Honestly, I'd be genuinely disturbed at the thought of person who would install base RJW but leave everything else at vanilla. I'd have to assume that they're on a level of depravity that I can't even fathom.
Try Maneater. Play as shark bite off the humans arms, legs and if you time it right their heads.
If no sexual content isn't a deal breaker you should be able to find that using the proper tags. You will need a decent gaming PC.

My point being that I browsed the mods and they weren't quite on par with MI's art and animations, which is understandable with RW being a game with more focus on management.
You are missing animation framework with animation packs,SAR nude body mod and some of it's patches and NAL's chibi face pack

No. The main source of negative morale is bad relationships with neighbors. Including mutual hatred and unrequited love. Just being sexually unfulfilled will not cause moral penalties.
