I don't do H-games (yet? I am trying to get into game dev... Although porn is probably a bad way to enter, so likely no H from me), but I do write H stories on occasion.
Some fetishes are kind of assumed. Most people can at least handle impregnation content. Very few people hear a girl going "cum inside! I want your babies!" and drop the work out of disgust the way they might if a turd slips out her bumhole on-screen. Foot fetish stuff tends to be a light turn-off to anyone who doesn't like feet, but not so bad as loli stuff. Etc.
Urine is surprisingly more often in the second tier, which is weird to me since scat is SOLIDLY in the deepest tier. But in both cases the response is mostly the same - warn the audience, and either focus the entire work on it, or avoid making it a major component.
I always start my stories with an idea of what's going to be happening in the story and I think the same should apply to game design. When I wrote Puella Magi Madoka Rapebait over on Akun I started it with an explicit mention of Madoka's age, and a scene where she's raped by monsters (in a dream). No sane person reads the title, then the rape scene, and goes "Why are you letting monsters rape lolis in my wholesome christian story?!"
May's does a good job of establishing this as a lightly bisexual loli story. You got that lesbian sister, the twink ass in the bushes, and May herself is naked on the opening splash screen. So it gets across pretty firmly that the story will include both lesbian loli, straight loli, and shota ass/dick. It's important, since it means basically nobody who loathes loli is gonna pick the game up and get slapped by their anti-fetish.
I don't really have somewhere to go with this though. The piss stuff isn't advertised as heavily but from the sound of it it's also not a central component of the game as a whole, and it does come early enough that it's not a massive piss-bending slap to the face. Just observing some stuff about creative smut writing, I guess, and how it interacts with different peoples' fetishes and anti-fetishes.

Yeah I think you have a very firm grasp on how content is communicated to the player/reader/etc. I don't know the right word to describe it, but you got it very right when you mentioned how the game frontloads all its major themes. Tho not something I'm intentionally exploring too much in this project, I think there's value in not just using the content to introduce itself instead of just listing the content beforehand (tho the tags are here and play their role), but also in having it as a surprise later. The reason I say this is because I only enjoy many of the things I like today because the story didn't warn me, and so I had no choice but to give it the benefit of the doubt and, before I knew it, it wasn't as bad as I thought. Though, of course, there's a spectrum to it, and I'd probably close any game that tries to surprise me with scat, even if I've grown to enjoy pee over time xD
At least with this game I'm not planning to challenge anyone's worldview on porn or anything, so every kink is introduced in a soft version of itself. This is partially so the player knows what they're getting into by just getting a small taste of it, and because I just like softcore and having things grow in intensity very slowly.

Funny enough, but I've published quite a few projects online, and pretty much everything always flopped (you know how algorithms work, you either pay, abuse search engine optimization or it will simply never show up on smbd's timeline), but my first attempt at an eroge has been the most successful so far. I guess the meme "sex sells" is real xD (Not that this game has sex per se, but... I love eroges and don't feel bad about it, but I've seen artists feel pretty dejected by this reality)
But don't let that dissuade you from making the art you want! If anything, when you get into game development, I'd be happy to have a look at it!
Anyway, I've started rambling too now. At the end of the, thanks for sharing your input! And dw, the game probably won't turn into smth it already isn't. It's just gonna branch out a little, explore a few new kinks, and slowly get a bit more extreme on the stuff we already have. This is my first time doing an eroge too, so this kind of feedback really helps me get an idea of what's the best way to add new content in a way that doesn't harm anyone's experience.
