I think the Capital is taking a while because it's supposed to act as a pseudo-dungeon or maybe just a full-on dungeon, with time passing as you travel around it, along with encounters that get more dangerous at night. It requires it's own framework and blueprint and isn't supposed to act like the beginner hub town. There are also things like waiting for art, proofreading, and implementing dialogue and events from outside collaborators. The creator is also adding more things to previous areas of the game in an effort to make them not feel empty.
Majalis has something like 288% more Patreon members and their lowest tier is three times as much as Threshold's lowest tier. This allows them to have two programmers, a dedicated writer, and a dedicated artist all working on the game. Whereas the artists for MGD are freelance and likely have to work on other commissions and the like and the people that volunteer to write scenes also have their own lives. Majalis also caters to a broader demographic with futa, furry, and a bunch of gay shit.
Please keep this stuff in mind and maybe play around with making your own game so you can see how hard it really is.
