Anybody who has the required skills can do it, if he likes the game. Why it's not leaked at the same time as the game? Perhaps because the author of the patch needs to have the game to make it, and it takes some time. About your last question, it's quite possible that Patreon watches what happens here, and elsewhere.

It's simple. Patreon doesn't accept incest. If you create a game with hidden incest and also create a patch to reveal it, you violate their rules. So the patch has to be created by somebody else.
[Edit] Of course the incestuous scenes are in the patch, the developper has nothing to do with that.

There's two images missing. I've modified the associated script to include them, took a bit of artistic license, and added some text too.

Take the linked script and add it to the game directory to see the missing images. If you want to revert back to the original, in the game folder, delete the v0.08_3_4_clean.rpy & v0.08_3_4_clean.rpyc files.
