The "NTR" is only the intro scene with Angel the nurse shagging the Doctor in front of the MC while he was in a coma. People lost their shit because Angel was considered potential LI, so it got the NTR tag.
Actually there's more than just that, If I recall around chapter 3 there was mention of a 3some that never actually happened while talking to the principle. Just seems way too inconsistent between the ntr and non ntr, mentioning things from other routes.
