You have the perspective you choose, so if you choose the Grandma option it skips to when they first visit. You get the thoughts of the characters and if other characters are not in the scene then only that character perspective. That said, it's planned that occasionally there will be scenes which have the other characters in.

So if you choose Grandma you won't have scenes where the boyfriend and girlfriend are shagging (but if you have either of their perspectives you will). If you play as the Mum you'll have scenes exclusive to her like her shaggin anything on legs, which the other characters wont have.

and the idea is that futa grandma will fuck GF.
what about mom? she will have lesbian sex with GF? or with grandma?

will there be other male characters? or the idea is that it will focus on this 4 characters?

All of that is in the dev notes but yes the Grandma will, mom will, mom is a futa. It will focus on these 4 but there may be secondary characters that are in the background that don't have any interaction beyond a scene. Like one of the flashbacks is how his mom dealt with a bully. Then that character doesn't appear again.
