I haven't touched this since the initial release... so I just started over from the beginning. Is it a bug that is forcing me to click multiple times to bypass each line of dialogue - it isn't every single line but a lot just from the prologue beginning?

In that case you need more points with her, you can use universal renpy mod to force that option though, it works fine for this game.

As has been stated, yes, need more points with Anna.
Also, not sure if it's a factor or not, there's an earlier choice, about NTS or cuckquean...
both of those choices have a flag variable cuckquean=True.
So, when getting the 2nd choice a little later, I think either of those choices will allow the cuckquean route, if you have the Anna points.

I think the issue goes beyond this. There are multiple games out there that seem to have had their netorare tags removed. I'm not sure if its devs revolting / playing some SEO game, a bug, or maybe the site admins have changed what qualifies, but this is not unique.

It’s up to you. The main focus of an AVN is obviously the NSFW scenes, but Sierra’s story and her demons, as well as Giulia’s, are still enjoyable even if you stay loyal to Anna.

I added the option to remain faithful to Anna because I know some players prefer her, and she’ll also have a unique ending once I reach version 1.0.

Yes, all the Routes are optional and depend on the choices made by the player.
