oooh, those lights. alrighty. once I'm on the true end path do I talk to fetus first and do the boss fight or do I talk to the npcs and do their quest first?

It's something very obscure for sure and it's possible. Wlafy (the one who translated the game) got it in one of his playthroughs but he couldn't reproduce it again.

There's 4 AFAIK. The regular "kill the final boss" ending has two slightly different variants depending on whether Fetus is alive or not. There are also multiple ways to get the second ending you mentioned but the ending is more or less the same.
