I will say, some chapters here or there without creampies are obviously fine (plus, we're only a short time removed from Laura's first ever creampie, one chapter removed from her first ever breeding play so it would make sense if she doesn't do it *every time* yet) but shouldn't the rule of thumb be that if people are going to keep turning down certain kinks at some point they just get less content, rather than those optional kinks get less "screentime" or you being forced to have a bunch of alternates?
And I'm pro-creampie so in this case I'm biased, but for the record if the fact I consistently turn down hints at MC eventually eating his own or a bull's cum (just not my thing) means that in X chapter I just get less content that's fine. There's a balance of course and you can't have a route where more selective playthroughs just have 0 pictures for several chapters but IMO the guiding rule should be that more kinks should mean more content on the whole.

For specific kinks, yes, you get less content.
But for things like creampies, I can't hardly skip whole scenes. That wouldn't make sense story-wise for Laura to not even have sex with you if you don't want to creampie her.
So, I have to choose between having two different scenes, or having Laura be a cum hungry slut that enjoys you cumming in her mouth. I alternate between them depending on the chapter.

While I understand its more work, I think it could generally be handled with it just being a change in the ending of the scene, a few lines of dialogue difference, and then one of two pictures depending on previous choices. This approach could actually work for a lot of the different kinks. No need to rewrite the whole scene around including or not including the kink, as long as the scene allows for it to be inserted without being a jarring transition.
As you revisit some of the preferences, you could also eliminate a lot of choices based on previous decisions the player has made, which.... idk if that helps you simplify things or not? Hopefully?
