I am guessing that the dev was probably looking for an impetus to push Rachel's boundaries further. Rachel's interaction with John would inevitably reach a plateau, which she would only give BJ or tit job as she clearly stated no sex except with her husband, unless there's a good reason for her to raise the stakes.

No, it's all very forced. Every conversation with John is always a thousand “please,” and she always refuses, only to give in at the end. In this case, there wasn't even that. yes, It's just random; it only appeared once. Agreed, she had one interaction with him at the birthday party and that seemed to more be a sympathy thing (like with John) then after seeing him at the store she jumps to this, when as you note, John goes through multiple steps.
