I also found that if Nesche has the nipple crucifix on her, and you run to ring it when being chased by the slave girls they'll get stunned (in a good way). I just thought it was kinda cool, not sure if it's new or not.

Jup, he rewrote the save/load system, entirely overhauled the restraint system and incorporated the status effect into the new restraint system.
These two things alone were already quite a lot of work, but there were many minor changes as well. Refractoring many of the classes, grouping them to namespaces and fixing lots of typos and wrong english names.
There was a lot of code cleanup under the hood, and it's much more readable now.

Yea I noticed that cuz I wrote a cheat menu for the previous version and it completely broke with that update and while looking into it I saw that everything was reworked. Managed to make my cheat menu working again though.
so is this the legit full game now? nothing else being added? full, final release?

The new additions to the game are wonderful, but sadly this current version is simply too buggy to play through properly. I feel we should be a bit more patient still.
My latest playthrough broke on the third level when I got caught, and Nesche turned invisible.
There was a list of other minor issues leading up to that too. She makes sounds as if she's out of breath, even while you're walking normally. The stamina bar no longer changes color when you use a potion for it either.
I'll be holding off until the next fix, before I attempt to play again. We've waited this long, so letting the artist cook a bit more isn't so bad.

Anyone Knows how to get the 3 slave girls Game Over. i Don't see any situation that could trigger that Game Over other than defeating Slave Merchant then get caught by the Slave Girls (however right now that's bugged cause if you do this you are stuck in stage 3 magic circle restrains that you cant remove and usually leads you to a Game Over.)

It really makes me wonder.
For some reason, the developer loves to re-use the keys. The same key is being used thrice:
First in the first stage.
The same key is then being used to open the door to the waterway (exit) in stage 3.
And again in stage 5 to get down to the masked orc.
I also heard that the chained key dropped from the masked orc is also being used in stage 6 again, though i havent actually played the stage yet, so i can't confirm.
This seem to be a possible root for several issues... Interestingly, making new items isn't even all that difficult in the game. Meanwhile, we still have plenty of items that aren't used or not finished (e.g. Oil, Cloth, Oiled Cloth)

Definitely a buggy release, I had trouble playing it. I noticed immediately in the scene viewer that the tentacle scenes are all messed up. Tentacle 3 plays the Tentacle 2 scene, and Tentacle 1 plays for both Tentacle 1 and 2. 3 is simply missing.
Where do i put the update files? Do i have to start a new game to get the right key on stage 5 ?
I tried to use it and still got the wrong key and cannot progress to stage 6
It's a regular BepInEx plugin. You need to install BepInEx 5 and put the dll into the BepInEx/plugins/ folder.

You also might need a new save. My patch only works when the orc is spawned, which I believe is the moment you first encounter him on the stairs giving a cure potion to the girl.
