Perry simply doesn't have any filter. But he also tries to support his friends in whatever limited ways he can come up with. He's genuinely concerned about Wade, and he even plays wingman for Ian at the very start of the game, getting the contact info from Lena for him, when Ian is too shy to ask for it himself.

Really, because in later convos it seems clear he was only getting Lena's info to stalk her profile while his "concern" for Wade is easily more to do with him hating Cindy than actually liking Wade. If anything he seems almost parasitic when it comes to Wade, actively wanting to break them up only so he can fuck about playing video games with Wade rather than any actual concern for Wade's happiness.
Yes, you need to make sure when you sleep with Holly, Ian has slept with Lena, but is not in love with Lena. The lena love flag comes from talking with Jeremy and saying youre falling for Lena.
