Don't need both to cheat. The easiest way is at Ivy's birthday party take the drug and blow either Mark or Mike. From that point both can be faithful. Ian confronts Lena at the beach and depending on which choice you make either one can ask for an open relationship.

I see... I'm trying to figure out where I went wrong most recently, and, looking at the walkthrough more closely, it seems like cheating on Lena with Cindy might be what fucked me over.
Am I understanding right that IanxCindy overrides any possibility for the open relationship?

Since Ivy and Lena are out of town, probably not. Though maybe some texting/sexting.
Pretty sure you can have Ian cheat on Lena with Cindy, but he has to deny doing it and flip the script on Lena by calling out her cheating.
Then, looking at the walkthrough yet again, I think my fuck-up might have been taking some of the choices that put me on the cuck route.
Looks like if you have a cuck score of 2+ you don't get to confront her about cheating at all, which makes sense I suppose. Doing a ctrl+F for "cuck" gives 150 matches in this document and I just don't think I have it in me to carefully monitor my cuck score throughout another legit playthrough. It takes enough mental energy minmaxing my cuck score in real life.
Respect to the dev for juggling hundreds of variables though. This really can't be an easy game to develop.

You could do the Ian/Holly version of trinity, right? No cucking Ian if he isn't with Lena until then..
To avoid getting a cuck score simply make sure Ian and Lena have sex regularly and do not have Lena tease Ian with footjobs, face sitting, and denying sex.
Pretty brutal of Eva to link the joy of facesitting with the psychological horror of cuckoldry. Come on now.
It's not fully linked to the cuck path, rather it progresses it if Ian is already on it. But it will always put Lena on the dominant path. Though the dominant path without the cuck path is very empty so doesn't really matter.

That scene happens on other routes, too, even if Ian is dominant toward Lena.
It just plays out differently if you have cuck points.
I realize that cuck points only start accumulating in Chapter 10, which means I only need to start paying attention to Ian not being too much of a bitch starting then.
So,
1. Ian and Lena need to be a couple
2. Ian's gotta keep his C-points to a minimum
3. One of them need to have cheated on the other while an official couple
4. If Ian fucked Cindy, he has to deny it
If I play with all of that in mind and still somehow don't get the scene trigger I'm ditching this game til full release in 6 years.

Not in the mood (dependent on Lena's disposition toward Seymour), or No Sex.
Compliment her, you can pretty much avoid the cuck route going forward.
If you choose one of the best, Ian's Chad level can prevent getting on the cuck route.
Lena does not have to explicitly cheat (the Seymour route is iffy) to end up on the cuck route.
Lena cheating just makes certain options/conversations happen...
The Holly Trinity route can be reached with Ian/Holly or Ian/Lena relationship. Ian cannot be a "cuck"...lol
As far as "corrupted" Holly, you can't let Holly be with Ivy. So, avoid that by keeping Holly's "want to be like Lena" to 2 or less going into the I think 2nd scene of the three of them at Ivy's place. Lena needs to take Holly home there. The first is where Lena kisses Holly, don't let Ivy do it.
Later on, at a club scene, Lena can encourage Holly to "go for it" with Mark, and Holly goes "slut" mode, but not under Ivy's "influence". Then, if Lena and Ian are a couple, you can get "slut" Holly in the throuple... I have one save route like that...lol
I think you can have a similarly "corrupted" Holly with the Ian/Holly route and get into the throuple, but I haven't specifically tried that route, yet. The key either way is avoid Holly and Ivy getting together at any point, that's one of the throuple killers.
After that Ivy's place scene, where Lena takes Holly home, Holly's "corruption" becomes "want to be like Lena" points. Accumulate the hell out of those after that Ivy's place scene, and you'll get a much more, Lena like Holly in the throuple or Ian/Holly couple.

It's very easy to end up with both of them being dominant of the other...so, switches. And face sitting is kinda stretching the definition of dominant here.
