Did an Ian/Holly run with a Posh Lena (fairly heavily into Seymour's pocket, doming Louise, playing with Ivy and Mark) and you can get the Holly Trinity, but you can also taint it by giving into Seymour in Rome. Can't mess around with anyone else though. And it's very clearly a planned path.

I got the impression that they did, albeit off camera... I did an Ian Cuck run and Lena disappears off to talk to Wade at the beach house and then asked Ian if he'd be interested in a threesome with Wade.

Lena is into Seymour more than anyone if you choose for your Lena to be head over heels with Seymour. I have no idea why this would be annoying, when it's basically the game acknowledging your choices and shaping the narrative to match them.

At this point EK has worked on the beach house sequence for literal months, i could definitely get/sympathize with the urge to move on and work on something different.

It happens even in great games like Baldur's Gate 3, the more true freedom you give the player the more possible weird combinations of choices you get. I am replaying BG 3 and I have encountered several NPC dialogues that didn't totally account for the timing and some combination of my choices and that's with a decent team working on the game and an exceptional post launch support.

My point is that Eva tries to cater to as much different player tastes as possible through the choices you can make but the risk/benefit for some paths like Lena/Wade and Lena/Perry doesn't make much sense and she would have been better just removing those possibilities from the game.

(slightly more seriously, this isn't quite accurate. If Lena only sleeps with Axel, but opts to stop her cheating on Ian in general, she isn't really flirting intentionally with Wade, she's just being playful. And prioritizes Ian for things like getting her sunscreen, taking pictures etc)

There's no specific limit or anything. If you had fuck buddies deal then it'll be dissolved if there's no feelings from both sides. If you do have a relationship, then whether it stays or not depends on decisions you make for both protagonists, regarding that.
