Likely nothing, because it's an easter egg the player can't normally select without having cheat mode on-- you can't get the checked attribute this high by the time the scene happens. Hence the Pikachu joke.
Although it seems the script has been changed more recently, and it's now possible to pick that option also if Lena went all in with Stalkfap. And the game records if Lena teases Perry. So, who knows. If Perry works at the cafe then his relationship with Lena automatically grows by quite a few points in the recent chapters.

Its funny, I started out playing this game really getting into the Ian-Lena relationship, I mean actually caring about them.
And then there's been some time between updates and I've been otherwise distracted and so when I finally tried out the newest chapters I started out on one of the...alternative routes where I had Ian with Holly and well...also fucking around with Minerva, Emma and finally Allison
Meanwhile Lena is domming Louise and confusing Holly.
So much for caring for Ian-Lena relationship (I will get back to that save too at some point)
Question is: can you form a lasting relationship with Minerva or Emma? Both are hot asf. (blonde Allison was a...healthy surprise too)

Minerva will break the relationship if Ian is dating Holly, so you would need to avoid that. Minerva is married, but she also starts taking care of herself in the earnest if she has affair with Ian, so who knows.
Emma is completely laid back about relationships, but iirc she isn't against it per se, just didn't find anyone who'd make her consider it. So, again, who knows.

I think in my both Ian and Lena are fooling around on the side, I still got locked into a relationship because Lena mostly had Louise, Ivy, and paid stuff on the side. What chapter was that decision, and how do you get the true open relationship?

What do you mean by "true open relationship"?
Lena/Ian are either a couple, or are just casually hooking up with no strings attached. The choice is made in Ch.9, the option to be a couple hinges on both Lena and Ian having expressed feelings for the other MC (using heart icon options) when given opportunity.
As long as Lena is in a "Heart-Symbol" situation with another girl, it unlocks the open-relationship path, provided Ian is high enough in lust. Just need to avoid Lena sleeping with Jeremy, which is a necessity anyway (Louise/Jeremy break-up) if you want Louise solo scenes & her heart option to be accessible to Lena. Open relationship discussion happens in Chapter 9.

Welp, I must have messed it up somehow, because Louise has a heart in her notes. Maybe needed to hit a higher threshold, or something.

So they have to keep it non-romantic? I figured just having multiple hearts for each character would have done it. Dang.
*Edit: And I suppose I should have phrased it as something like 'the beginning of a romantic open relationship,' but that would be me projecting a bit. First playthrough all I was going for was a Ian/Lena/Holly throuple, but that blows up in your face hard. This could never be a harem game, but I could see them seeing each other as primaries, and fucking around on the side.

That is one of the most hypocritical, but exceedingly prevalent cliches in AVNs. If it's a male MC they can often veto it, but I can't think of an instance where it was considered cheating. Just finished Summer's Scent for the first time, and wow, that's some double-standard shit. Not game ruining, but both funny-haha and funny-weird at the same time.
