Gillian foreshadowing only really happens for the player if they have Lena fuck Mike, and even then Ian is not privy to any of that info, so it'd still be a big shock for him specifically.

My guess on my first playthrough was Robert, just because he definitely comes off as needy any time Lena interacts with him after they've slept together. No reason why he wouldn't be the same with Holly. It certainly could be Clark, or Mark, though I doubt it's Stan.
