The best way to do this, of whether or not Cindy cheats with Axel, is being dependent on Ian's actions. There are choices in which Ian could ignore Axel becoming closer to Cindy like ignoring Axel touching Cindy in inappropriate places instead of commenting about it. I believe there is even a relationship stat between Axel and Cindy where ignoring Axel's advances adds points to that stat.
